Registration of Deaths
REGISTRATION OF DEATHS OUTSIDE SRI LANKA
The following documents should be submitted for registration of deaths of Sri Lankan nationals who have passed away in the United Kingdom or Ireland.
Duly perfected application – (Please use Form B 11 for registration of a death occured within 3 months, and Form B 15 for registration of a death occured more than 3 months ago). Form NQ obtained from the UK Home Office to confirm that the applicant had not gained the British Citizenship at the time of death, if the applicant has been issued Indefinite Leave to Remain (ILR), Permanent Residence Card (PR) or Settlement one year or before. Death Certificate issued by the authorities of the country where the death occurred (United Kingdom/Ireland) with a photocopy. OTHER INFORMATION Payment should be made in cash at the counter or by Postal Orders/Bank Drafts written in favour of the “Sri Lanka High Commission” if application is sent by post. Personal cheques and credit cards are not accepted. Fees sent by applicants living in Ireland should be by Bank Draft. All original certificates will be returned after perusal. If the certificates and documents are to be returned by post, a self-addressed prepaid special delivery envelope to the weight of 500g should be sent along with the application. The special delivery cover should not be stamped with a postage printout issued by the post office for same day post. Applications sent without prepaid special delivery cover will be retained until the applicant collects it personally or sends a self-addressed prepaid special delivery envelope. Note: The High Commission will not take any responsibility for documents lost in the post. If the applicant is unable to visit the High Commission to collect the Certificate/ documents personally, they may authorise another person, in writing. The person who collects the documents on behalf of the applicant should produce an dentification document and the original receipt issued by the High Commission. SUBMISSION OF FORGED CERTIFICATES OR ANY OTHER DOCUMENTS AND MAKING FALSE DECLARATIONS IS AN OFFENCE. SUCH CASES WILL BE REPORTED TO RELEVANT AUTHORITIES. |
